People with mental disabilities getting older – access to care through the study of medical devices accessibility in retirement homes.

A major challenge today is the access to care for people with mental disabilities getting older (PDO). French care system is divided in two part. On one side, old people face with problematics linked with the aging (like Alzheimer disease for example). They are welcomed in specialized institutions like retirement homes. On the other side, people with mental disabilities deal with difficulty of communication or expression and are welcomed in other specialized institutions. A new population is appearing : people with mental disabilities getting older. These people cumulate difficulties in relation with their mental disabilities and the aging process. Several problematics are emerging What is the best support for PDO who are at the crossroads ? How can we guarantee them a good access to care ?
The main problem is the lake of knowledge in terms of needs, wishes and expectations by professionals caregiver. To answer this problematic, we propose a public and dynamic data base with 18 personas : 5 personas of PDO, 11 personas of professional caregivers and 2 personas of familial caregivers. Personas allows caregiver to have a better knowledge about these elements. They are requirement specifications to design accessible tools.
Professionals are not trained to take care and support this population in its care pathway and in its life course. We propose a serious game in the form of board game with a personalization software. With the software, professionals can create scenarios for the game. They are based on their own experience. Scenarios are a simulation of a crisis situation. Professionals must think about their practices and improve them in order to improve them to guarantee a good access to care for PDO.
Tools like medical devices and technical helps are not adapted to the characteristics of PDO. We have evaluated two medical devices : medicalized bed and rollator. We have also worked on three technical helps : Boccia (a physical technical help), UbiSmart (a technical help based on the Internet of Thing way of process) and Amikeo (a numeric technical help). For each of them, we suggest requirements of adaptation and use.
It is very difficult for professionals to access information they need or it is  very time-consuming for them. We have developed a decision support system in form of a site web. It helps professionals to select adapted tool for resident according its characteristics, history, needs and wishes. It also helps to follow its use with residents.
All these tools have been co-developed with professionals in an human centered design and universal design way. They are distinct from each other but complementary. Each tool can personalized according the resident (its characteristics, wishes, expectations, needs, history of life and care pathway). Their used can be extended to other frailty group of population people ageing,  people with mental disabilities, ...
For the next steps, family will have to be included in the development of the tools.
